You need: a review open in the player, with the Notes panel open (select Notes on the right edge) and a comment in it. To write one, see Add a comment on a frame or range.
Type your reply in the box that says Add reply, mention with "@"... To mention someone, type @ and choose a name.
Press Enter, or select Reply.
Your reply appears under the comment. A line such as "2 replies" shows how many there are; select the comment to read them. To answer one reply in particular, point to it and select Reply there. The reply box stays open until you select another comment, and the hover buttons on that comment, including React, stay hidden meanwhile.
Point to the comment and select React, the smiley with a plus.
Choose ๐, โค๏ธ, ๐, ๐, ๐, or ๐ฅ. For any other emoji, select More emojis and search.
To take your reaction back, select the same emoji under the comment.
Each emoji appears under the comment with a count, and yours is highlighted. Select an emoji someone else added to add yours. You can react to a reply the same way.

revis.io shows Couldn't save your reply, and the reply stays under the comment marked Not saved, even if you collapse the thread. Select Retry to send it again with your original text and attachments, or Discard to remove it. Retry works while the tab stays open: reloading the page loses an unsaved reply.
